is the way we practice church discipline scriptural? What could we do better?
When I read about it, it appears that if you have a disagreement or your brother wrongs you that is when you confront them, then take two witnesses & only after that bring it before the ecclesia/church.
the other place Ive read about it is in one of the letters from the Apostle Paul, he mentions a man who is sleeping with his fathers wife(something terrible)... and to "not even eat with such a one". but then later there is restoration.

I have observed that people can be put under discipline for smoking, divorce, marrying outside the church & that excommunication is practiced for murder, fornication & adultery. Are there more things that people are disciplined or excommunicated for?

Why does the congregation have to know if someone has done these things? Doesn't God forgive there sins?

If they confess, should the confession & repentance be sufficient. The first commandment is to love God and love your neighbor as yourself.

Hebrews 6

4For it is impossible for those who were once enlightened, and have tasted of the heavenly gift, and were made partakers of the Holy Ghost, 5And have tasted the good word of God, and the powers of the world to come, 6If they shall fall away, to renew them again unto repentance; seeing they crucify to themselves the Son of God afresh, and put him to an open shame.
(note: "to renew again unto repentance")

2 Peter 2
15Which have forsaken the right way, and are gone astray, following the way of Balaam the son of Bosor, who loved the wages of unrighteousness;
(note: "forsaken the right way" and "gone astray")

2 Timothy 2
12.......... if we deny him, he also will deny us:
(note: this cannot be reversed)
(the biblical meaning of "deny" can be referred to as:

To refuse, reject and renounce God, Jesus and the Holy Spirit (after receiving the Holy Spirit) by giving allegiance to a person, thing, country or state.
or
To go back into the world and sin and disregarding God, the work of Jesus, and choosing to disobey the Holy Spirit in your life.

Hebrews 12
17For ye know how that afterward, when he would have inherited the blessing, he was rejected: for he found no place of repentance, though he sought it carefully with tears.
(note: "found no place of repentance")

Hebrews 13
4Marriage is honourable in all, and the bed undefiled: but whoremongers and adulterers God will judge.
(note: God will judge. Its not given for man or a church to be able to judge, forgive or reinstate particular sins members have committed. Thats Gods work that He will conduct on the judgement day)
